HBUHSD staff may attend any of our school based vaccination clinics from 3:30-5:00.

All students will need an ID and parent consent form signed. Further, the Orange County Health Agency Nurses will require a questionnaire to be filled out before students can be vaccinated.  Vaccine Information Sheets, required questionnaires and consent forms will be available at all the sites and on websites prior to the clinics and at the clinics.

I look forward to providing vaccination clinics to our students and staff so that the obstacle of illness does not impact what we do best, educate our young people so they can have successful futures. I want to thank all staff and community members involved in making these clinics possible as it is a large undertaking in these busy days.
